1. Insurance: All professional services are billed directly to the patient, who is personally responsible for payment.
As a courtesy, we will complete any necessary forms or reports to assist you in obtaining insurance benefits and can usually submit claims electronically. However, our services are not provided with the expectation that insurance companies will cover all fees.
Fees are individualized for each patient and reflect our level of training, skill, and experience. Except for insurance carriers with whom we have a direct relationship, all insurance matters are solely between you and your insurance provider, not between the insurance provider and this office
